Turn years of medical records into one indexed binder

Records from several providers, in one indexed PDF with each document labelled Tab 1, Tab 2 and continuous page numbers, so anyone reading it can find a page by its number.

Optional. Printed on the cover.
Optional.
Leave at 1 unless you have been told to continue from an earlier volume. Any other start means the printed numbers no longer match the PDF's own page numbers.
As uploaded keeps each PDF page its own size. A4 or US Letter puts every page on that paper, shrinking anything bigger to fit.

The finished PDF, made from an invented example

This is the real output of this tool, made from an invented example by the same code that makes yours. The people and details in it are fictional.

How it works.

Records from different providers arrive with their own page numbers, their own layouts and no common order. This puts them into one file in the order you choose, numbers every page from front to back so anyone can be pointed to page 212, gives it an index whose entries link to each record, and bookmarks each one. What an insurer, a benefits office, a solicitor or a court wants a medical bundle to contain and look like is for them to say, so check that before you build it. This does not read, summarise or run OCR on your records.

Step 1

Select your records

Select all your PDFs and photos together. Photos go on their own page and are shrunk to fit if they are bigger than it, never stretched. PDF pages keep their own size unless you set Paper size to A4 or US Letter, which puts every page on that paper and shrinks anything bigger to fit.

Step 2

Name it and check the index

Give it a title and a reference if it has one. The preview shows the index with the page each document starts on, so you can check the running order before paying. It shows the index, not the pages themselves.

Step 3

Download the finished binder

One PDF: cover, index, every page numbered continuously at the bottom centre, a small label box in the top right corner of each document page, and bookmarks down the side. The number and label are drawn on top of the page, so anything printed in those two spots can be hidden.

Trust, with receipts.

No invented review scores, no badges, no “trusted by thousands”. Here is what is actually true, and you can check every line of it.

Questions, answered.

Will it build a chronology or summarise my records?
No. It assembles and indexes what you give it. Reading records and deciding what they mean is clinical and legal work, not formatting, and a tool that pretended otherwise would be doing you harm.
My records are scanned and not searchable.
They go in as they are. This does not run OCR, so a scan stays a picture. Run OCR first if you or whoever you are sending it to needs to search the text.
Is the index clickable?
Yes. Every row in the index is a link to the first page of the document it names, and each document has a bookmark named from its file name, so you can jump around the binder without scrolling.
Can I add to it later?
Build it again with the new documents included. There is no subscription. The price goes by page count, so a binder that grows past 50, 200 or 500 pages moves into a higher price band.
How do I control the order of the documents?
Select every file together in one go, because picking a single file starts a fresh preview with only that file. Documents go in the order your browser hands them over, and browsers differ, so check the running order in the preview before you pay. Numbering the file names, like 01 GP records and 02 Hospital discharge, makes it easy to check. The index names each document from its file name, number included.
Will you tell me what to put in it?
No, and that is deliberate. Putting your documents in order and giving them an index is a formatting job. Telling you what to include, what it proves or what an insurer or court will accept is medical or legal advice, and we are not doctors or lawyers. This assembles what you give it and nothing more.
What happens to my file?
It is deleted within 7 days, along with the finished PDF. It is used to make your file and nothing else. Nobody reads it.
Do I need an account?
No. There is no signup, no subscription and no password. You upload, you see the result, you pay once if you want it.
